All WARP families are required to complete this form before any on-hill activites begin. Each athlete needs to have his/her own form.
Participants must sign the form in addition to a parent or guardian.
The signed release waiver must be completed by all WARP athletes and racers visiting from other mountains.
Complete this application to become a member of the United States Ski & Snowboard Association.
This form must be completed for each racer at each race that is a sanctioned PNSA or USSA race.
